Insulin is a natural, glucose-lowering hormone that many people with diabetes use to control blood sugar. In people with type 1 diabetes, extra insulin makes up for insulin that is not produced in the body. People with type 2 diabetes may need to take insulin if their blood sugar is not adequately controlled with other medications.

Insulin is produced in a manner similar to the insulin produced by the human pancreas. These synthetic insulins can work from a few hours (rapid-acting insulin) to a day (long-acting insulin). They are usually injected using a needle or pen.

As anyone with diabetes can tell you, managing the disease with insulin usually means regular visits to the doctor’s office to adjust dosages, monitor blood sugar levels, and check for complications.

That’s what Carmen Smith did for six years without health insurance and without a primary care doctor. He bought the insulin over the counter at Wal-Mart.

“We can’t say, ‘I need insulin,’ with a trench coat and a hat,” says Smith, who lives in Cleveland. “The clerks usually don’t know it’s a big secret. They’re just like, ‘Do we sell over-the-counter insulin?'”

Smith didn’t learn from a doctor that he could buy insulin that way. In fact, many doctors don’t even know it’s possible. When she doesn’t have insurance to pay for doctors’ appointments or medications, Smith asks Wal-Mart if she can get vials of the medication without a prescription. To know the dosage, he used the amount given by his doctor several years ago.

It was a way to survive, he says, but no way to live. It was terrible that he couldn’t get the dose or the timing right.

“It’s a quick high and then a low,” says Smith. “The down side is, you feel sour. You feel lifeless. You feel pain. The cramps are so intense — you can’t walk, you can’t sit, you can’t stand.”

The availability of over-the-counter insulin poses a real conundrum. Smith’s experience shows that without training or guidance from a healthcare provider, predicting the best dose and timing for a patient from version to version of insulin can be dangerous. On the other hand, being able to buy some when he needed it might have saved his life.

There are two types of human insulin; one by Eli Lilly and the other by Novo Nordisk. These versions of the drug are older and take longer to metabolize than some newer, prescription versions; they were created in the early 1980s and prices range from $200 to $25 a vial, depending on where you buy it.

Dr. Jorge Calles, an endocrinologist at MetroHealth Cleveland State Hospital, is concerned that some people are self-medicating with some form of insulin.
